Effect of serum lipid profile and its association with cardiovascular diseases in patients of Type 2 diabetes mellitus: Rethinking targets?

Abstract
Background: According to the latest 2016 World Health Organization data, an estimated 422 million adults are living with diabetes. The coexistence of diabetes mellitus (DM) and its cardiovascular complications is considered as metabolic syndrome (MetS). One of the major cardiometabolic risk determinants of MetS is diabetic dyslipidemia.

Aims and Objectives: The objectives of the study were to evaluate the lipid profile and its derangements in diabetic patients and predict cardiovascular risk in patients with Type 2 DM (T2DM) using Framingham risk scoring (FRS) for men and women.

Materials and Methods: FRS, a gender-specific algorithm, was used to estimate the global 10-year cardiovascular risk. The score was classified into two risk categories: High risk (if >10%) and low risk (if <10%).

Results: A total of 55.1% males and 9.5% females were having atherogenic dyslipidemia. All the females who were dyslipidemic fell into the high-risk category based on their Framingham risk scores whereas 93.75% of the dyslipidemic males fell into the high-risk category. There was a strong positive correlation of Framingham risk score values with age (r = 0.795), systolic blood pressure (r = 0.806), diastolic blood pressure (r = 0.817), and triglyceride levels (r = 0.705) whereas there was a moderate positive correlation with BMI (r = 0.598) and HbA1c levels (r = 0.564). There was a strong negative correlation with high density lipoprotein levels (r = −0.754). A total of 37 (74%) patients had an estimated 10-year Global cardiovascular Risk >10% (high risk) whereas 13 (26%) patients were having risk <10% (low risk).

Conclusion: We found that overlapping risk factors were involved in the etiopathogenesis of T2DM and cardiovascular disorders such as age, gender, BMI, HbA1c levels, hypertension, and dyslipidemia. We noted that diabetic dyslipidemia led to a higher risk of adverse cardiovascular outcomes. More than two-thirds of the patients fell into the high-risk category under FRS.
